Okay....where to start....
I'm not too familiar with all your players but I can give you some tips regardless.

As to your formation and instructions, I play the same formation in my FC United save which has gotten me back to back promotions from Vanarama to the Championship now in 2019. If you're gonna play fullbacks make them automatic not support, you have good experience in Trip and Evra so they should be able to judge when to get up the pitch or defend well.

Make 2 different versions of this tactic, 1 for Home and 1 for Away games....for away leave it as counter, take off the overlap instruction and put shoot on sight instead of work into box, always make wingers or inside forwards support because on atk they stay too high up and wont come to defend often, set width to slightly wider (or whatever the next bar is called) take off pass into space as this will mostly result in giving away the ball to their defenders, set closing down to the max setting and turn play out of defence on for any tactic you use...and ALWAYS use higher tempo for counter style of play.

For Home tactic, push the DM up into your midfield to make it a 4-3-2(wide)-1, i dunno about your game but in mine Will Hughes is a great DLP, with decent defensive stats but much better on offence, you're wasting him as a DM, you need to buy a good BWM, look for good tackling, marking, work rate, strength, positioning and determination, and use him as the deeper BWM in your away tactic or the central BWM in the 3 man midfield of your home tactic, that way your BBM and DLP(Hughes)/AP(Aketxe) can push up a bit more while the BWM stays back and deals with any breakaways. Good instructions for Home tactic are, Standard(for top 10)/Attacking(for weaker teams) mentality, i always try use fluid team shape, especially for the higher leagues, higher tempo, the highest closing down setting, normal defensive line, retain possession, shorter passing, play out of D, exploit both flanks, be more expressive and work ball into box. When you assemble a better squad after a year or two of PL money, then think about using run at defence, but now your attacking players are prolly gonna get dispossessed by the better quality defensive players, I find that short passing around the box gives them more trouble than one guy running at his man anyway.

As to your squad, seems solid enough to keep you up, but you need to bring in a quality keeper if you can, cos your main guy doesnt seem to be doing it, either hes still adjusting to PL or just not good enough, 6.5 avg for a keeper doesnt cut it for me, main stats to look for are one on ones, reflexes, positioning and handling, a great shot stopper will save SOOOO many goals for you. Oh and make your defenders CD defend, BPD is nice if you have players like Aldeweireld, but as a newly promoted team with a comparatively weaker squad you dont need your CD throwing away possession trying to pick out a wonder pass. Give your striker the shoot more often player instruction, and your defensive mids the shoot less often one, if you have a good inside forward give them a shoot more often too.

So there are a few things you can try, hope it helps, lemme know how it goes
