Need help with a tactic

So I am in my 3rd season in my Fiorentina save. I created my own tactic (one of the first times I try to create a tactic on my own) and the first two seasons went very well-the first season I finished 3rd,and in the second season I finished 2nd,reached the 1/4 finals of the champions league and played in the cup final. Despite the decent results, the tactic always had its flaws, and now at the start of the 3rd season the flaws in the tactic are more obvious than ever, contributing to a dreadful start of the season. I managed to get only 12 points in the first 10 games. The biggest problem for me are both the winger and the inside forward. They have performed absolutely dreadful since the start of the save, usually ending the majority of the games with a 6.2-6.3 rating. The other problem is the defence- I feel that it should be doing better, especially when I have great defenders like I do at the moment. The only positive is the midfield three, which always performs well.
Can someone have a look at it and give me some advice to make the tactic better? As I said, its one of the first tactics I make and it is not perfect.

Changes I have tried so far:
Changed the striker from PF to CF
Tried support role on IF and W, but ended up changing it to attack again
Switched the Mezzala from attack to support
Tried different player instructions
Switched from geggenpress to control possesion

It's actually not a bad tactic....

Some things I would change though are as follows....

One of your three forward players to a support role, your DMC to a defend role to protect against a counter, one of your DC's to normal defender rather then BPD. Turn off Pass in to Space as you are short passing so passing to feet makes more sense.

I also don't like counter press & counter together but many do, just be aware that a team better than you could exploit this.
