I've played FM for years and never came across this in any of the other versions.

Keep getting beaten by a huge amount usually 3 goals, yet team performances are averaging 6.5-8 in each game.

Even in the cups to lower league opposition. So frustrating, is anyone else having this?
do ur players suffer from over confidence. or do u put much focus in attacking ??
Can you tell the team you are managing ?

Also, you need to tighten up defense a bit i guess, probably set match prep to defending set pieces.

try 451 for a while, if you are playing with same formation every time, chances are that you will get beaten..
are u seeing something wrong tactically? or is ur team making stupid and unrealistic mistakes all the time?

at times it could just be that u need to defend deeper or more narrow or something else in that fashion that can help. usually u should see something in the matches, where most of the goals come from or the making of the goal.

ur defence might not be making many mistakes but it could be ur midfield just being all over the place for an example.

u need to find these details and try to counter it with some tactical changes.

experimenting usually helps.

really? i thought the 'familiarity score' of using the same tactic was supposed to counter this?

Yes you train your squad for 3 different tactics, and ideal situation is that your squad is well familiar (fluid) in all compartments for all three tactics, its a fact that if you use same tactics in every match, you will lose or draw many games in the latter part of the season as other teams start preparing counter tactics !

thats friggin awesome....

i saw in one of the games by joannes3000 (http://www.fmscout.com/q-2173-An-old-face-in-a-familiar-place.html) i think he used a straight up 442 for pretty much 20 seasons and was very successful with it.
I play either a 4-3-1-2 formation or 4-2-3-1 and set all my defenders to focus on defence and i have 1 of my centre backs as a stopper. When playing 4-3-1-2 i set my midfielders all centre midfielders or 1 left mid 1 centre mid and 1 right mid dependin on what players i have. I would suggest having 3 centre mids and set 1 to focus on defend 1 as support and 1 attack. Then set you attack midfielder to trequista or advance playmaker dependin on who your have. Up front i have 1 poacher and a complete forward. Now 4-2-3-1 is my favourite formation set all defenders to defend and have a centre back as stopper. Now depending whether you want to attack or defend you want either two defensive midfielders in front of your defenders or have two centre midfielders whichever you decide set 1 as a ball winning midfielder and the other to defend. Your attack midfielders are simple just have a left and right winger set to attack and your attack midfielder as a trequista or advanced playmaker also set to attack. Finally set your striker as a poacher and hopefully your problems will be sorted. If you need anymore help let me know and i will upload some screen shots of my tactics and go into more detail. Hope this helps.
Along with the tactical advice, maybe lower the expectations in press conferences and team meetings if you think that your players are suffering from over-confidence or complacency. I know how you feel, Carling Cup 2nd rounds are killers with every player seeming to think too much of themselves.
Agree with romiljain you should train your squad up so they know a minimum of three formations. I would also try a 4-3-2-1 formation with wingers and three centre midfielders.
