Need help for my QPR continuous poor form.

My team now lying 5th in the league. But the our recent form was really poor. I know that using same tactic will contribute to this, but I have 2 tactics which I regularly use. The players all in bad morale. I dont know what to do, our game plan just so poor. Defences gave away goals so easily. Any tips?

change the formation, put 5 in midfield if you arent already, and call a team meeting to raise morale
