So, people could say its a tactical problem and most times it is but I would also like to add that it may not always be tactical. try to manage your matches, not just leave it to the AI and expect wins. watch the match and know when someone isn't playing well or needs a role change. also it can be a morale problem, winning is a habit and so it loosing. increase team morale with team talks and motive players too after training weeks. know when to play cautiously in a match and when to be positive, you can only do this if you watch matches and see how open you are positively or how closed off your team is cautiously. Too cautious against a weaker team is not to your benefit and being too positive against a stronger team can also leave you vulnerable. its football manager, you will learn more on the job, don't just rely on downloaded tactic, you miss the fun in the game.
