Can someone make me a banner please?

Team= Rochdale AFC
Words= "Rochdale: Rising Revolution"
League= Barclays Premier League (you could include a small premier league logo if you wish to :) )
Colours= blue, and white if you need a secondary colour
Other= please could you include the club logo on there too?
